Most people exchanging gifts would already be family and prolly help already if they are willing to. While groceries is not a resource to shy away from it's not really the point of Christmas in a relative sense. Giving grocery stuff to homeless sure, but most people are giving gifts to family members and prolly not just giving idealistic gifts to people who are struggling. Your concept is valid in that we should help those in need but I feel kind of misses the point of the gift giving you are referring to. You're trying to combine charity work with familial tradition and bonding, which in a lot of cases might be kind of redundant or otherwise insisting a resource that most people celebrating Christmas in a gift giving fashion don't really need.

How do you get better at gift receiving when most of the stuff is crap you'll leave in your basement for the requisite amount of time you need before tossing it without guilt or worrying they'll ask about the gift. I don't want groceries either I guess. A card is fine.

This is highly individualized. Those who cannot afford basic necessities would definitely benefit from groceries/grocery store gift cards. Maybe you can request this from those who usually send you gifts? Or return or sell gifts bought for you and get the cash to buy what you need? For others who already have basic necessities, this would be an unusual gift. Maybe something home baked, would be appreciated, if you are inclined to give a food gift.
